The declaration of illness or functional impairment that a person must provide to obtain or renew a licence or to pay the amounts prescribed in section 93.1 of the Highway Safety Code (chapter C-24.2) must refer to one of the following health problems:(1) the wearing of eyeglasses or contact lenses for driving;
(2) eye disease;
(3) loss of hearing including deafness;
(4) vertigo;
(5) heart trouble;
(6) arterial hypertension (high blood pressure);
(7) functional impairment of the neck, spine, an arm, hand, leg or foot;
(8) mental illness or psychiatric problems;
(9) alcoholism;
(10) drug use or addiction;
(11) epilepsy;
(12) nervous system illness;
(13) loss of consciousness, convulsions, syncopes (other than epilepsy);
(14) diabetes;
(15) lung or bronchial disease;
(16) illnesses other than those listed in this section;
(17) intake of medication affecting the nerves such as stimulants or tranquilizers.
